Pulp therapy for pediatric patients

The pulp is located in the center of the tooth and consists of nerves, tissues, and blood vessels. The main function of the pulp is to provide the tooth with oxygen and other nutrients to keep it healthy. Teeth that are subjected to traumatic injury or decay can cause pulp exposure, which leads to severe pain and swelling. Our pediatric dentists treat and restore both primary (baby) and permanent teeth. Although primary teeth eventually fall, they are needed for speech, proper chewing, and correct alignment of permanent teeth.

Signs of pulp injury or infection

  • Regular unexplained pain
  • Nighttime pain
  • Swelling around the infected tooth
  • High sensitivity to warm and cold food/drink temperatures
  • Looseness of the affected tooth

How is pulp therapy done?

The pediatric dentist performs a visual examination and digital x-rays to identify the location and amount of pulp damage. The two main forms of therapy are pulpotomy and pulpectomy. Using pulpotomy, the diseased pulp is removed without affecting the pulp root to be replaced by an agent to prevent bacteria growth and infection. Finally, the tooth is capped with a crown to minimize future fractures and strengthen the tooth.

Pulpectomy is used if the entire pulp is involved from the crown all the way to the tooth’s root canals. In this case, the whole pulp is removed and the canals are disinfected and packed with biocompatible (reabsorbable) material. If the affected tooth is permanent, a non-reabsorbable material is used. Finally, the tooth is capped with a crown to support and strengthen the tooth.

Management of teething

The eruption of primary teeth usually begins around 4-8 months of age with the eruption of the lower incisors, and is complete at around 30-36 months of age when second primary molars erupt. However, the timing of tooth eruption varies by as much as six months.

Pit and fissure sealants

Grooves or pits and fissures on occlusal surfaces of primary and permanent posterior teeth trap food debris and promote the presence of bacteria due to their deep and morphological complexity thereby increasing the risk of developing cavities. Effectively penetrating and sealing these surfaces with a dental material like pit-and-fissure sealants providing a physical barrier that inhibits microorganisms and food particles accumulation can prevent lesions.
